Are you sure these are just “Most Embarrassing Movie Posters?” They seem like Most Embarrassing Movies to make PERIOD!!
If I’m not mistaken, the head that “wants to kill” was a black supremacist, and therein was the big part of the conflict.
This movie’s really rather tame, and more notable for having Casey Kasem as a star, though with only one head on his shoulders.
The only thing vaguely embarassing about this one is that it looks as if the poster was created before the two actors were cast, since neither head looks like the people in the movie. In fact, I’m having a difficult time discerning which one is suppose to be black. Ray Milland must have been happy, but Rosie Greer probably was left shaking his head in wonder.

I was confused by that at first too, T Edward Fox, but we were both thinking of the better known classic, “The Thing With Two Heads” (http://www.imdb.com/title/tt0069372/) which actually seems to be a rip-off of the earlier “Two-Headed Transplant” (http://www.imdb.com/title/tt0067245/).
I thought it was an early John Candy/Charles Bronson buddy picture made while Candy was on his way up and Bronson on his way down.
